Peter C. Loeser, MD
Dr. Loeser is the owner and operator of Crossroads Family Medicine and Pediatrics, which he founded in 2008.
Raised in New Hampshire, Peter received an undergraduate degree in business administration from the University of New Hampshire, completed
his pre-medical training at Harvard University, and received his medical degree from Dartmouth Medical School.
For several years, Peter was the medical director at a federally funded
community health center, and prior to opening Crossroads Family Medicine and Pediatrics, he operated a consulting company specializing in electronic
health record data analysis.
Dr. Loeser is board certified in family medicine.
Sarah C. Medeiros, APRN
Sarah received a Bachelor of Science in biology from Stonehill College and her MSN through the MGH Institute of Health Professions in Boston. Sarah previously worked for DHC in Concord for
more than 13 years and has been a Pediatric Nurse Practitioner since 1997.
Sarah is board certified in Pediatrics.
Richard L. LaPage, APRN
Richard L. LaPage is a native and lifelong resident of New Hampshire. Rick received his Bachelor of Science in Nursing from Regis College in Weston Massachusetts in 2014 and his Master of Science as an Adult/Geriatric Nurse Practitioner from Regis College in 2015. Rick has worked in multiple areas of the healthcare system from the pre-hospital 911 system to long term care and hospice setting.
Rick is a board certified AGPCNP-BC and a member of Sigma Theta Tau International.
Sarah Depelteau, MSN, APRN, FNP-C
Sarah Depelteau is a board-certified family nurse practitioner with the American Academy of Nurse Practitioners. Sarah earned her undergraduate degrees in both Exercise Science and a Bachelor of Science degree in Nursing from University of Massachusetts Lowell, MA and a Master of Science in Nursing from Rivier University, Nashua, NH. Sarah has experience working as a full-time Family Nurse Practitioner in Primary Care. In addition, Sarah previously gained extensive experience as a registered nurse in adult critical care, cardiac-telemetry, pre-op/PACU, surgical medicine and school nursing before transitioning to primary care. As a nurse practitioner, Sarah's philosophy of care is to provide individualized care through shared decision-making, acknowledging each patient's unique situation and health care needs. Sarah is committed in educating her patients and providing comprehensive high-quality evidence-based care. She believes in an honest and trusting relationship with her patients and desires to care for her patients holistically. A native of NH, Sarah enjoys her time with her husband and two children as well as their fun-loving cat named ginger. In her spare time, she loves being with her family, visiting Newfound Lake, hiking, reading, decorating, cooking and exercising.
